Non-convulsive epiletic seizure after electroconvulsive therapy session

چکیده

Introduction Electroconvulsive therapy (ECT) is a procedure performed under general anaesthesia involving triggering an intentional brief seizure through small electrical currents the brain. The anaesthetic depth should be adequate prior to shock and measured with BIS, processed electroencephalogram (EEG) monitor. Adjusting hypnotic dose allows decrease ictal threshold thus improve response treatment side effects. Objectives Our goal detecting elements such as spontaneous epileptiform activity after ECT without tonic-clonic spectral density matrix (SDM). Methods patient: 87-year-old woman, diagnosed F20.2 catatonic schizophrenia antipsychotic since her youth. She has required multiple hospital admissions due psychopathological decompensations until starting monthly maintenance sessions in 2014. Since then she no new have been required. Results Images 1D 1E shows SDM, spectrogram of EEG. X axis show time (minutes), Y frequency (Hz) Z energy or intensity that bands replaced by colors: warm colors (red) reflecting high electroencephalographic cool (yellow, blue green), low activity. 2 3’s EDM electroconvulsive We observed initial awake patient, colours practically all bands, including beta band (13-30 Hz), characteristic waking states. Around 9:50 induction occurs, increases slow frequencies (red alpha, theta delta), plus increase cold beta, disappearance brain frequency. asterisk reflects EEG discharge, followed postcritical state exclusively waves amplitude (delta some theta) absence other (blue colour alpha bands) around 9:57. At about 10:00 there abrupt appearance (warm colours) delta, mainly, clinically reflected patient absent disconnected from environment, but New which predominates, little activity, warmer reappear progressive recovery wakefulness. Image: Image 2: 3: Conclusions Psychiatric pathology can observe changes EEG, correcting stimulus anesthetic appropriate trigger anesthesia. Disclosure Interest None Declared

Convulsive Status Epilepticus After Electroconvulsive Therapy

Status epilepticus is a very rare complication of electroconvulsive therapy. A case of a 58-year old patient with catatonic schizophrenia is reported who developed a status epilepticus after ECT. At a neurological critical care unit the status finally could be stopped. The origin of the epileptic status lastly remained unclear (German J Psychiatry 2013; 16(2): 81-83).

Seizure threshold in electroconvulsive therapy: I. Initial seizure threshold.

We measured initial seizure threshold by means of a structured stimulus dosage titration procedure in a clinical sample of 111 depressed patients undergoing brief-pulse, constant-current electroconvulsive therapy (ECT). Initial seizure threshold was approximately 60 millicoumbs (mc) (10 Joules) on average, but varied widely (6-fold) across patients. Initial seizure threshold was predicted by fo...

Unruptured intracranial aneurysm presenting with epiletic seizure.

Intracranial aneurysms are frequently present with subarachnoid hemorrhage. Less often they produce suggestive symptoms of cranial nerve dysfunction or intracranial tumor when very large. Their association with epilepsy has rarely been reported; such concurrence may not be a coincidence. When the patient presents with epileptiforme attacks the presence of an intracranial aneurysm is rarely cons...

Variations of blood glucose level in non-diabetic patients before and after Electroconvulsive Therapy

Introduction: Application of electroconvulsive therapy (ECT) for the induction of generalized seizure was first introduced in 1938. It is applied for disorders as severe depression particularly for cases which do not respond to drug therapy, or during pregnancy when medication is detrimental to the fetus. The debate on the effect of ECT on blood glucose level is still contradictory. However, st...

Comparable seizure characteristics in magnetic seizure therapy and electroconvulsive therapy for major depression.

Electroconvulsive therapy (ECT) is highly effective for treatment-resistant depression (TRD); however, its use for less severe forms of depression is somewhat limited by a lack of control over current spreading to medial temporal lobe memory structures, resulting in various cognitive side effects. In contrast, magnetic seizure therapy (MST), which uses high frequency repetitive transcranial mag...
